Second series of Resource, Education, And Collaboration in Health (REACH), a program developed by SSM Health's Coordinated Care Network (CCN) will begin Wednesday April 26 at the Golden Age Senior Center in Centralia.
The Coordinated Care Network (CCN) works closely with area hospital patients who have chronic illnesses and are having a difficult time managing their health. A fairly common factor the CCN found was isolation; many patients lived alone or did not have much support or good knowledge and information about food choices and healthy living.
A six week series program was developed that educates community members about how to lead a healthier life, including such topics as stress reduction, motivation, goal setting, self-management and much more. In January the first Resource, Education, And Collaboration in Health support group (REACH) group began meeting at Heritage Woods. The six week series was a huge success and they continue to remain active and motivated.
CCN Director, Darren Dunahee, said, "We began the REACH series because we've recognized several needs within our own CCN population, and the community as a whole." The goal of REACH is to educate the community on various aspects of healthy living and overall well-being. They provide support to participants who are working towards their own health care goals, and assist in the formation of peer-led support groups.
"We've noticed that many of our older patients, especially those with chronic conditions, tend to live more isolated lives. This can have a significant impact on their mental and physical health," added Dunahee.
Bringing the REACH series out into the community the CCN has given people the tools they need to be socially and physically well. Bringing people together to form smaller communities within the larger one, helps them to find those social connections that drive increased engagement and support for healthier lifestyles.
